New Client Information
Thank you for choosing Grove City Veterinary Hospital for your pet’s health care needs. It is our mission to protect and strengthen the human-animal bond by providing compassionate and knowledgeable care to you, our client and your beloved pets.
Attached is a copy of our New Client form. To lessen the waiting time in our lobby please bring the completed form with you. You can also e-mail it back to us prior to your appointment at: Info@grovecityvet.com. If you are unable to complete this form in advance, please try to arrive 15 minutes early.
Please bring your pet’s previous records with you or they can be faxed in advance to: (614) 875-5644 or e-mailed to: firstname.lastname@example.org
We will do our best to make your pet’s first visit a happy one. We will have treats available for your pet during their examination. We encourage you to bring your own pet’s favorite treats if he or she is finicky or has a dietary restriction or allergy.
Cancellations: Please allow 48 hours advanced notice, when possible, for appointment cancellations. If you will be more than 15 minutes late; please call our office to reschedule at (614) 875-4321. Thank you.
Our hours (by appointment) are:
Monday: 9:00 a.m. to 8:00 p.m.
Tuesday: 8:00 a.m. to 6:00 p.m.
Wednesday: 9:00 a.m. to 8:00 p.m.
Thursday: 9:00 a.m. to 5:00 p.m.
Friday: 9:00 a.m. to 5:00 p.m.
Saturday: 8:30 a.m. to 12:00 p.m.
We accept the following payments: Visa, Master Card, Discover, Care Credit, ScratchPay or cash. Sorry, we do not accept checks.
